The following are forms used for financial aid purposes at CNM:
- Appeal form (158 kb) - applies to Pell, Loan and Student Employment
- Deferment Authorization form to charge books and supplies is available online through my CNM
- Direct Deposit form (98 kb) - used for grants, loans, and scholarships to be deposited into a bank account
- Financial Aid Cancellation form (54 kb) - used to cancel financial aid
- Financial Aid Needs Analysis - used for verification of income based on the financial aid application information
- Housing Status form (103 kb)
- Stafford Loan Revision/Cancellation form (141 kb) - used to make changes in Stafford Loan amounts and terms
- Veterans Class Schedule Certification form for a) Montoya, South Valley and Westside Campuses (29 kb) or b) Main Campus (45 kb) - used by eligible veterans, guardsmen, reservists, and dependents to request class certification for Veterans Administration educational benefits
- Veterans Affairs Proof of Course Completion form (96 kb) - used for documenting course attendance
- Student Employment Application - used after the financial aid file is complete, the student can use this form to apply for a job
- 2009-2010 Dependent Verification Worksheet (190 kb) - used by a dependent student selected for verification by CNM. Students whose Student Aid Report states they have been selected for verification should disregard that statement.
- 2009-2010 Independent Verification Worksheet (186 kb) - used by an independent student selected for verification by CNM. Students whose Student Aid Report states they have been selected for verification should disregard that statement.
