Parking Services is has a number of forms that we are frequently asked for. To help serve the CNM community these forms have been placed online for easy access.

CNM Reserved/Gated lots Registration Form - This form is required to be returned to cashiers at time of permit purchase for the reserved and/or gated parking lots. One permit per person.
Isotopes Student Vehicle Registration Form - This form is required to be returned to Parking Services for a free Isotope card permit.
Isotopes Employee Vehicle Registration Form - This form is required to be returned to Parking Services by faculty or staff members for a free Isotope card permit.
CNM Student Vehicle Registration Form - This form is required to be returned to Parking Services for a CNM Security student vehicle registration parking sticker that will allow students to park in the non-fee lots located on all campus except the Isotopes lots at Main Campus.
CNM Employee Vehicle Registration Form - This form is required to be returned to Parking Services for a CNM Security employee vehicle registration parking sticker that will allow faculty and staff to park in the non-fee lots located on all campus except the Isotopes lots at Main Campus.
CNM Parking Services Appeal Form - To appeal a citation this form must be turned into the Parking Services Office prior to the 16th calendar day of citation issuance.
