Supervised Field Experience Application

This is the initial application for Supervised Field Experience (SFE).  Supervised Field Experience (EDUC 2190 or SPED 2390) is to be taken as the final class in your program of study. The application is due by October 1 for consideration to complete SFE in the Spring.  The application is due March 1 for consideration to complete SFE in the Fall. This application is closed at 5pm MT on the specified date. 

Before filling out this document, you must:

  1. Have read though the SFE Application Information, Checklist and Timeline e-mail
  2. Printed or saved a copy of the SFE Application Checklist Timeline
  3. Briefly looked through the CNM SFE Information Document

(All of which have been e-mailed to you.)

By filling out this application you agree that you have done the above and are registering your intention to take the SFE class in the following term.

Agreement #1   I have read though the SFE Application Information, Checklist and Timeline e-mail, printed or saved a copy of the SFE Application Checklist Timeline and have briefly looked through the CNM SFE Information Document.

Agreement #2   I understand that the Supervised Field Experience class (SFE) is to be taken as the last class in the Program and that preference will be given to those students who have completed all of their required coursework.

Agreement #3   I understand that participation in the Supervised Field Experience class (SFE) requires that students have passed certain exams (see SFE Application Checklist/Timeline for details) prior to beginning their field work.

National Evaluation Series (NES) Assessments

If you have not taken or passed the appropriate NMTA Content Knowledge exam(s), you will now need to complete the appropriate NES Assessment.

Licensure Area(s) for SFE   Please indicate the Licensure Area(s) in which you plan to conduct your SFE. Note: To have your SFE count for more than one licensure area your placement must meet the requirements for both areas. For example, licensure in Elementary and Secondary would require a placement in 7th or 8th grade.

Please note that in order to assist you with placement, you will be required to provide our office with your Field Experience Log(s), a current resume and a Letter of Introduction that we can send to potential cooperating teachers and administrators.  This Letter of Introduction should discuss you, your strengths and weaknesses, and what you are looking for in a classroom. Please submit these documents in Word format to  no later than March 1 for Fall placement or October 1 for Spring Placement.

After you complete this form, be sure to submit the following documents:

  • Signed Agreement to Provide Placement
  • Cooperating Teacher Information Sheet
  • Signed copy of the Contract for SFE
  • Field Experience Log(s)
  • A copy of your “Authorization to Student Teach” or district specific clearance.

 

Please turn all paperwork in at the same time.  Incomplete or partial paperwork will not be accepted.  Paperwork can be submitted electronically to or brought to the education programs offices at CNM's main campus.
