HR has relocated to the TM Building

The Human Resources Department has relocated to the Ted Martinez (TM) building at Main Campus (formerly the Administration - A Building). The offices are located in the lower level of the TM building in room 104. Business hours are 8:00a - 5:00p.

Form 1095 (Affordable Care Act) Tax Forms now available

The Internal Revenue Service (IRS) will use the information reported from your 1095 to determine whether you may have to pay a fine for failing to comply with the Affordable Care Act. Think of the 1095 ACA tax form as your “proof of insurance” for the IRS.

To view a tutorial on how to sign up to receive your tax forms electronically, click here.

While the information on these forms may assist in preparing a tax return, they are not required for 2015 tax preparation. Like last year, taxpayers can prepare and file their returns using other information about their health insurance.
